The client
- NHS Great Yarmouth and Waveney Primary Care Trust (PCT) works in partnership with local councils, clinicians, service users and other stakeholders to deliver primary, secondary and community health services to a population of 235,000.
- Its vision is for the population of Great Yarmouth and Waveney to have the fastest improving health in England.
- The Trust’s total funding allocation for 2008/09 was £345 million.
What we did
Flag was commissioned to lead the development of a new website, aimed at supporting people living with long-term conditions in the Great Yarmouth and Waveney area. Such people might have chronic respiratory conditions, or arthritis or mental health needs. The concept is aligned with recent Department of Health directives for patient care, as, through combining a high level of community functionality with a comprehensive library of resources, it empowers patients to help manage their condition by themselves and to help each other.
Working with web development partners Eduserv and contacts from across the Trust, the new site enables users to personalise the content to long-term conditions of their choice, and contains a community forum to enable visitors to exchange ideas, information, advice and opinions with each other. The site also has links to the latest health, wellbeing and lifestyle news and events, a directory of local health-related services and support groups, prominent links to services such as NHS Direct and NHS Choices, and ‘share this page’ social networking functionality.
